Clinical Analysis of Surgical Diseases on Neck


Abstract
During the past 3 years and 7 months from Jan. 1974 to July 1977, 240 cases of surg¡þical diseases on neck confirmed by pathological examination and operative findings were treated at CNU Hospital.
The results were as follows;
1. The most common surgical disease on neck was inflammatory origin, which comprised 39.2%, most of them were tuberculous lymphadenitis (25.8%).
2. The surgical disease was distributed at all age group and female predominated over males by-1.4 : 1.
3. The most frequent chief complaints were mass(100%), palpitation, headache, weak¡þness in the thyroid disease, and mass(93.7%), pain and tenderness(21.3%), mass and discharge in the non-thyroid disease. Bleeding was frequently noticed in the traumatic lesion.
4. Solitary nodules were 38 cases(89%) and 20 cases (42.6%) were located in Lt. lobe in thyroid lesions. Site distribution and number in the non-thyroid neck mass were as follows, unilateral distribution were 146 cases (80.7%) and 76 cases (41.9%) were located in Rt. side on neck and 138 cases(76.2%) were soiltary in number.
5. The size of mass of majority was medium size in the thyroid mass and small size in the non-thyroid mass.
6. The most frequent duration was 1^¢¥2 years(34966) in the thyroid disease and below 3 months in the non-thyroid disease (mostly within few hours in traumatic lesions).
7. Primary site were known in 18 cases(58. 1%) with metastatic lesion.
They were 7 lung cancer, 5 thyroid cancer, 3 stomach cancer, 2 uterus cancer and 1 pharynx cancer. Primary sites were unknown in 13 cases (41. 9%) with metastatic lesion. 8. Main sites of neck injuries were 5 cases (41. 7%) soft tissue, 3 cases (25%) vascular, 2 cases (16.7%)muscle, and 1 (8.3%) nerve, 1(8.3%) larynx.
9. In 48 cases of malignant tumor, radical operation was performed in ,8 cases. Biopsy, chemotherapy, excision and irradiation were performed in the remainder.
